The Outreach Health care team provides assistance with activities of daily living to complex disease management.
Outreach Health provides home health care including
- Mobility assistance
- Personal care
- Food preparation and feeding
- Environmental services
- Escorting client to appointments
- Shopping
In order to be eligible for in-home care, a functional need threshold, also referred to as a medical need, must also be met. This could include the client requiring help with mobility, transferring from a bed to a chair, toiletry, bathing, doing laundry, or meal preparation.
Your case manager will send us the referral information, and we will reach out to you to schedule a time to meet you and family members that are part of your care team.
This first visit is performed by a qualified health professional to discuss all aspects of care needed and answer any questions or concerns you may have. We will then make sure to match you with the perfect caregivers and create a care plan to fit your desired schedule.
If you are just beginning your search for home care, one of the first questions might be around home care cost. Services are usually paid directly by the person receiving care (private pay) or through long-term care insurance or Medicaid. Outreach is already partnered with most insurance companies, as well as local Area Agency on Aging (AAA) and VA programs.
The Every Baby Matters Mission at CHRISTUS Santa Rosa Hospital – New Braunfels provides mother and baby with an in-home visit by a registered nurse following the days of delivery.
The nurse provides parenting education and a medical assessment for mother and baby. The EBMM strives to give parents the tools they need to allow babies to thrive in strong, healthy families.
Certified Nurse Midwives provide healthcare throughout ALL life stages, not just during pregnancy and birth.
Available services include:
- full physical with a breast exam
- PAP smear
- sexually transmitted infection (STI) treatment for you and your partner
- pelvic exam
- lab work
- family planning counseling
- prescriptions
- referral for diagnostic testing
- teen education
I work collaboratively with an OB/GYN, and should any conditions require more specific attention, a consult or referral can be easily made.

I believe the miracle of birth is a normal natural process and every person has the right to choose where and how they give birth. My goal is to assist you with your transition into new parenthood.
Throughout the birth process, you will receive labor support as needed, birthing person and fetal assessments will be completed, and you will have the freedom to move, eat, and drink as you desire. Water birth can be accommodated while you enjoy the comfort of the warm water. Skin-to-skin bonding with your infant immediately after birth and breastfeeding/chestfeeding support without interruption is offered. Constant evaluation and observation of the birthing person and baby during the postpartum period for several hours is provided all the while allowing for family bonding.
Postpartum visits are scheduled between 24-48 hours after the birth, an optional 5-7 day office visit, a 2-week office visit for the baby and birther, and a 6-week visit for the birther. During this period I will be available for any questions or concerns you may have. Certified Nurse Midwives can care for newborns up to 28 days of age, With this being said, it is encouraged you arrange a primary care provider for your baby as soon as possible. Please contact San Antonio Nurse Midwife for more details and about formulating your specific plan of care.

Hospice is an appropriate course of care for terminally ill patients in the last six months of their lives, at a time when curative treatments are no longer an option.
Hospice caregivers help patients manage their illness and remain as comfortable as possible in their environment. The goal of hospice is symptom management rather than treatment or recovery. Hospice care complements the guidance of a patient’s physician.
Hospice helps ease the burden of families caring for patients during this time.
When is someone ready for Hospice care?
- At the recommendation of their physician
- When diagnosed with a life-limiting illness and when symptoms are no longer responding to a curative treatment
- If you feel that your loved one may qualify for hospice services, please call one of our offices to discuss hospice eligibility and specific health conditions.
Care Locations
Our goal is to provide hospice services in the environment that best meets the needs of the individual patient, caregiver, and other family members. To that end, we provide care in whatever setting our patient calls home:
- Personal Home – When possible and desired, we help our patients remain at home during their terminal illness. Whether care is provided in the patient’s home or a caregiver’s home, our team of professionals and volunteers makes sure that the appropriate equipment and supplies are placed in the residence so that it is as comfortable and well-equipped as possible.
- Nursing, Assisted, and Independent Living Facilities – When caring for a patient who resides in a nursing home, an assisted living, or an independent living facility, our hospice team complements the services and skills of the facility staff and works with that staff to care for the patient. In addition, we offer our full spectrum of support services to the patient’s family.
The hospice care team that we bring to our clients and their families includes:
- Nurses
- Nurse Practitioners
- Social Workers
- Therapists
- Patient’s physician
- Hospice medical director
- Home health aides
- Bereavement counselors
- Chaplains
- Volunteers
New Creation provides support through a network of professionals, trained volunteers, and community resources.
We want you to know that you are not alone. Let us walk with you through life's traumas and transitions and with the help of the Holy Spirit, guide you to the other side toward health, healing, and wholeness.
We offer:
- Counselors and referrals
- Hospital and homebound visitation
- Bereavement and funeral planning assistance
- Pastoral care support groups
- Suicide prevention
- Prayer support

Often, people cannot attend Mass due to illness, surgery, or other incapacitation and thus cannot receive the Eucharist.
Our Homebound Ministry brings the Eucharist to them in their homes during these times. The duration of their need may be short-term or indefinite. The days and times for visits may vary according to need. They can be very flexible but are usually once per week and are arranged between the assigned minister and the parishioner in need.
The ministry is a wonderful opportunity to bring Jesus in the Eucharist to those who could not otherwise receive Him, to help them keep in touch with our Church Community, and to form special relationships. Training is available.
If you are feeling called to this ministry and have any questions, please email Mary Morgan at [email protected]
If you, a family member, or a friend need this ministry, or if you know of someone who is, please contact the Church office at 830-779-3131.
Texas HCS (TX HCS) is our company that provides HCS services (Home and Community-Based Services) in our Beaumont, Harlingen, McAllen, and San Antonio service regions…offering 3 and 4-person group residences, along with the entire array of available HCS services since 1993 in San Antonio and McAllen, since 2000 in Beaumont, and most recently since 2007 in Harlingen.
We take great pride in the development of each of these companies, each serving many people with great success.
Companions for the Journey is an organization dedicated to serving the homebound, the sick, and those grieving. We will bring Holy Communion to the shut-ins as often as you like and pray with the family caretakers.
We offer our friendship by praying with you and for you. If you are grieving over a loved one, take comfort in knowing you are not alone.
We also have a Gathering of those Grieving on the 3rd Thursday of every month. We offer a forum to voice your feelings, and topics to cope with, and speak with a counselor if you wish. At the request of the family, a priest will visit the sick person and give the Sacrament of Anointing of the Sick.
Helping patients understand what happens after their physician has diagnosed a life-limiting illness is a standard educational component when consulting families. We educate clients about facts and myths of hospice care so they have a better understanding of the condition and how to deal with it.
Hospice Services Include:
- Aide Services
- Bereavement Counseling
- Medications as Related to Hospice Diagnosis
- Nursing Services
- Physician Services
- Social Worker Support Services
- Spiritual Care Services
- Volunteer Services
Hospice focuses on caring, not curing and in most cases care is provided in the patient's home. Hospice care also is provided in freestanding hospice centers, hospitals, nursing homes, and other long-term care facilities.
The hospice team develops a care plan that meets each patient's individual needs for pain management and symptom control.
Among its major responsibilities, the interdisciplinary hospice team:
- Manages the patient’s pain and symptoms;
- Assists the patient with the emotional psychosocial and spiritual aspects of dying;
- Provides needed drugs, medical supplies, and equipment;
- Coaches the family on how to care for the patient;
- Delivers special services like speech and physical therapy when needed;
- Makes short-term inpatient care available when pain or symptoms become too difficult to manage at home, or the caregiver needs respite time; and
- Provides bereavement care and counseling to surviving family and friends.
The Meals on Wheels San Antonio Friendly Visitor Program is a volunteer-based program whose goal is to provide companionship and a meaningful friendship to a homebound citizen in Bexar County. Friendly Visitors are matched with a homebound senior based on similar interests, availability, and gender preference. Friendly Visitor volunteers can visit in person with a homebound senior and do things such as play games, take a walk, or chat over coffee or tea. The volunteer could also call the client every week to chat about how they are doing. This position is great for individuals who have a heart for helping out others and who would love to spend some free time with a person who needs a Friendly Visitor. Too often, seniors who are no longer able to leave their homes can start to feel isolated, lonely, and depressed, which may worsen existing health conditions.
Friendly Visitor volunteers help to alleviate concerns by visiting a homebound senior to provide much-needed companionship. Just a simple conversation can make all the difference to a lonely person.
Volunteers interested in becoming a Friendly Visitor must:
- Be at least 18 years of age
- Pass a criminal and sex offender background check
- Complete a one-hour training/orientation
- Be a little flexible in caring for recipients' need for quality time
- Be able to relate positively and warmly to older adults
Friendly Visitor volunteers are matched with older adults based on personality and interest so that a bond can easily be developed. Once paired, volunteers are expected to provide friendship and conversation. In some cases, Friendly Visitors offer minor assistance where appropriate (e.g. walks in the neighborhood, computer assistance, etc.).
For more information about receiving services, or to volunteer, please contact Friendly Volunteer Coordinator, Jillian Huskin by email or call 210-735-5115.
Any Meals on Wheels client who needs companionship services, may be suffering from depression, or has experienced the loss of a loved one, may be eligible for service.

The mission of the homebound ministry at St. Anthony de Padua is to bring Jesus, both in the Eucharist and in the gift of our presence to our sisters and brothers in the community who are unable to be present at the Eucharistic table because of illness or disability.
This extends to those in nursing homes, retirement centers, hospitals, or private homes-wherever this need is present. We try to meet two times a year with the Pastor, so that the members can discuss and share any situations or ideas that can improve our ministry. Please call the rectory if you are unable to attend Mass and would like to receive the Eucharist.
There are four Nursing/Assisted Living or Independent Living Homes in the St. Anthony de Padua area that we visit regularly. Volunteers visit with the residents and on special occasions will provide gifts and goodies for them.
Each week there is a Communion Service or a Mass at the various homes and the Eucharist is brought regularly to all Catholic residents.
Our parish Nursing Home Ministry serves:
- Alamo Heights Rehab and Nursing Home
- Stone Brook Manor
- Emeritus at Lincoln Heights (Brighten Gardens)
- The Forum at Lincoln Heights
