Hill Country MHDD Centers provide compassionate care and support to individuals with intellectual and developmental disabilities by providing services that offer hope, support, and recovery.
Our Service Coordinators are trained in person-centered thinking and person-centered plan facilitation, which means they have learned skills to support individuals to achieve their goals at home, school, work, and in the community.
The Service Coordination team assists with:
- determining eligibility for services;
- enrollment into programs;
- coordinating and monitoring ongoing services;
- placing individuals on the Home and Community-Based Services (HCS) and Texas Home Living Interest lists;
- helping students transition from school services to community-based services; and
- aiding families in finding residential services for children and adults.

The purpose of the Single Family HOME Program is to expand the supply of decent, safe, affordable housing to eligible low-income households. CCSCT is qualified to also provide assistance with set aside funding for Disaster Relief and Persons with Disabilities.
TBRA provides the following services to eligible individuals in local communities:

Education Services
- The Center on Independent Living has been involved with educating people with disabilities and life skills in the community as funding allows. We are constantly seeking ways to provide this service at no cost to the people who need it. This could include family members as well.

- Our clients need transportation as well as those who are currently living independently in the community. This specialized service will enable the Center on Independent Living to enhance freedom for people with disabilities.

Home Modification Services
- The Center on Independent Living has been involved with home modification services since the 1990s as funding allows.
Relocation Services
- Funded by the Texas Department of State Health and Human Services (TXHHS), the Center on Independent Living helps disabled Medicaid recipients who are in nursing homes achieve independence in the community.
ATC owns single-family and duplex group homes. Residents are matched with staff and fellow residents who meet their needs, interests, and abilities.
Staff members serve as personal care assistants, teachers, and friends offering a warm and accepting environment.
Activities in the home include learning how to be a contributing household member, meeting personal grooming needs, enjoying leisure activities, and participating in community events. Outings to grocery stores, malls, parks, restaurants, movies, and exercise (walking, biking, swimming) increase social skills and well-being.

HCS facilities are 3- or 4-bed group homes that are available through the Home and Community Services Medicaid waiver program and are certified and licensed by the state. Individuals must be diagnosed with an intellectual disability and have an IQ of 75 or below with a related condition.
We also serve individuals who live on their own in an apartment setting.
Private pay option: Individuals with developmental disabilities who do not have Medicaid funding can pay privately for services in a 3-bed or 4-bed group home. We also serve individuals who live on their own in apartment settings.

Adults with developmental disabilities can choose to live in a foster home with a family member or other individual as their foster care provider.
RMI contracts with the foster care provider to assist with daily living skills, personal care, transportation, and other activities that allow the client to live as independently as possible in a safe and supportive environment.

Funding for foster care is covered by the Home and Community-Based Services (HSC) Medicaid program.
Individuals who do not qualify or are waiting for HCS funding may pay privately to live in a foster home.

ICF facilities are 6-bed group homes funded by Medicaid. These homes are certified and licensed by the state to provide a full range of training and care to individuals with intellectual and other developmental disabilities.
ICF group homes include:
- 24-hour nursing support
- Therapy
- Nutritional and dietary supports
- Behavior supports
- Medical and dental support not funded by Medicaid
- Active training to master new skills
This program is for individuals with intellectual and other developmental disabilities (IDD).
All services are paid by Medicaid and personal income is managed by the provider, representative payee, or the client. If the client works, a percentage of earnings is refunded to Medicaid. SSDI (Social Security Disability Income) funds can continue with a percentage repaid to Medicaid.

RMI's Semi-Independent Apartment program gives adults with intellectual and developmental disabilities the ability to live in the community with more freedom and more responsibility. Residents in this program lease their apartments and pay their expenses. They may choose to live by themselves or share an apartment with a roommate.
At San Mateo Apartment Homes, Waters at Elm Creek, and other apartment communities in San Antonio, RMI residents work, play, shop, and worship with others in their neighborhoods. With ongoing support from RMI staff, residents can meet the everyday challenges of living independently.

Murray Manor, Inc. is one of three apartment complexes in San Antonio sponsored by Mission Road Ministries that provide affordable housing and services for adults with intellectual and other developmental disabilities, but need minimal supports.
The apartments are Housing and Urban Development (HUD) subsidized apartment complexes, designed exclusively for adults with intellectual and other developmental disabilities, and are supervised by the Suzanne Smith Management Company.
The apartments have one or two bedroom options with a community room for gatherings and a laundry room.
- Adult with an intellectual developmental disability
- Low-income
- Not under guardianship
- Independent in self-care, self-medication, self-evaluation in emergencies
- Minimal assistance required in home safety, kitchen/household management, clothing maintenance and community mobility
- Active employment or participation in competitive employment, sheltered employment, vocational training or a day activity center.
Permanent Supportive Housing assists and empowers families to live as independently as possible. SAMMinistries has a permanent housing program that assists families with whom one member of the family has a disability. SAMMinistries also manages 100 set-aside housing vouchers to help families find permanent housing in the private rental market.
Case managers work with adult participants to achieve the following goals:
- Remain in (or exit to) permanent housing
- Increase skills and/or income
- Achieve greater self-determination
- Achieve financial stability (reduce debt)
The individual or family:
- must be "literally homeless" by HUD definition (living in an emergency shelter, on the street, or in a place not meant for habitation), or fleeing domestic violence.
- in some cases must be "chronically homeless" by HUD definition (a single person or head of household with a disabling condition who has either experienced homelessness for longer than a year, during which time the individual may have lived in a shelter, Safe Haven, or a place not meant for human habitation; or experienced homelessness four or more times in the last three years).
- must be referred by the community's Coordinated Entry system. Call (210) 207-1799 to get that started.
For some projects, client pays 30% of their income toward rent, however, no income is required.

This is a chance to help make a difference by opening your home to an individual with a developmental disability.
A Host Home Provider offers a community-based living alternative where individuals or families open their homes to people with developmental disabilities who qualify for services. Teaching, assisting in personal and professional growth, and providing recreation opportunities are additional responsibilities of becoming a home provider. Pay is based on the supported person’s level of need. Community Options will provide training.
A Host Home is more than just another housing option. The home’s primary caregiver provides training, assistance, and supervision. Support can range from community integration to daily living skills, such as personal hygiene, food preparation, and instruction on social skills and problem solving.

Community Options develops specialized housing and supportive services to meet the needs of individuals with disabilities. We buy, build, renovate, and furnish the kind of homes that we want for our own families. We promote individualized ownership and empower those we serve to choose the setting that best meets their needs.
We provide community-based residences in safe and attractive neighborhoods and communities. Many of our residences are barrier-free ranch homes with two to four bedrooms. Individuals living in these homes receive 24/7 direct support care from highly trained staff who understand the individual’s Habilitation or Support Plan.

Our supervised apartment program is for adults with IDD who need minimal support.
Mission Road Developmental Center’s three apartment communities are Housing and Urban Development (HUD) subsidized complexes. The apartments are designed exclusively for adults with IDD and have one or two-bedroom options, community rooms, and laundry rooms.
Like other children’s foster shelters, Respite Care of San Antonio's Residential Children's Homes operate 24/7/365 to provide protection and urgent essential needs (shelter, food, clothing, transportation, and care) to children in crisis removed from their homes due to abuse and neglect.
Unlike other shelters, our program is dedicated to serving children with intellectual and developmental disabilities and complex medical conditions. This vulnerable population needs extra resources to overcome the consequences of medical neglect and inadequate care.

Habilitative Homes (Hab Homes) is a licensed Residential Treatment Center providing 24-hour care to children with severe intellectual and/or developmental disabilities, often due to a pervasive developmental disorder such as autism.
Hab Homes provides an enriched, predictable, supportive environment in which the children can learn and grow in normal, healthy ways developing skills in self-care, self-direction, self-discipline, self-awareness, responsibility for decisions and choices, and building skills necessary for living as independently as possible as well-adjusted, productive adults in our society.
At Hab Homes, the environment, the programming, and the staffing are always child-centered.
Admission Criteria
- Children must be between four and 17 years of age at the time of admission and may remain in care until adulthood
- Children will have a diagnosis of IDD or borderline intellectual functioning with some exceptions
- Specialized or Intense LOC (or equivalent level of need)
Morningside offers long-term care services at two state-licensed communities for individuals with illnesses, injuries or functional and physical disabilities who require full-time nursing care.
Our staff specializes in skilled nursing care, long-term care, and rehabilitation services as well as hospice and respite care. Our medical director and nursing staff provide a wide range of health care services for those with multiple needs and diagnoses including, but not limited to:

This low-income senior citizen
The Southwind City Locals program is designed for men and women of abilities who desire to live independently within the community and have the necessary skill set to do so with minor assistance.
City Locals live in our partnered, affordable price, apartment complex on the Northwest side of San Antonio. Here, they maintain their lease agreements, while enjoying the fellowship and community of living alongside other Southwind City Locals.
City Locals can attend the Southwind Day Program, should they desire, where they may participate in community impact team mission work and attend Life Groups and Worship Gatherings.
Other aspects of the City Locals program include:
- Roommate match (Locals choose their roommate from a pool of potential life-long friends)
- Community apartment living centered around the community-strong concept
- Personal Local Advisor (case management/advocacy/State & Medicaid provider supervision)
- 24/7 on-call support
- 24/7 on-call wellness advisor
- Minimum of twice-weekly check-ins
- Weekly environmental checks
- Bi-weekly support from an Occupational Therapy Intern
- Independent living guidance
- Bus/transportation training
- Optional financial management with bi-weekly budget meetings

Fees:
- Non-refundable program initiation and application fee: $150 (helps us cover the costs associated with apartment paperwork, job search, government-based fees, etc.)
- City Local Program Fee: $350/month. Your monthly fee covers all of the services listed above as well as access to the Southwind Local Society.
- Rent: Apartment application fees and deposits are typically around $150). Monthly rent is approximately $520/per person for a two-bedroom apartment and $890/per person for a one-bedroom (Locals opting to live in a one-bedroom must pass an evaluation for living alone before applying).
