- Is a Texas Resident living at home or with a family member
- Is eligible for Medicaid in the community and is not enrolled in any other waiver program
- Does not exceed SSI-related criterias for income and resource limits
- Was diagnosed with a condition or disability, other than an intellectual disability, before age 22 that affects their ability to function in daily life
- Demonstrates the need for habilitation and case management services
- Meets the level-of-care criteria for placement in an ICF/IID
