Our Medical Social Workers provide various services to our clients, including counseling and locating community resources to help them recover quickly in the comfort of their homes. We advocate for our clients and their families to cope with their social, emotional, environmental, and financial needs caused by their illnesses and treatments.

We work with other social service and home care providers to help you with your healthcare journey and promote healthy lifestyles, prevent diseases, and overcome barriers to quality services to help you achieve the best possible outcomes.

Medical Social Services can evaluate psychosocial issues including social, emotional, family, and resource factors impacting the patient’s condition, rate of recovery, and /or ability to follow the medical treatment plan as well as the patient’s adjustment to new/exacerbated illnesses or injury.

Requires physician authorization and certification of homebound status and medical necessity. Available to individuals, including older adults and individuals with chronic conditions or disabilities, residing within the service coverage area (Bexar County and surrounding counties).

As long as you meet the Home Care eligibility requirements, Medicare will cover your services, and in many cases, Medicaid and most private insurance can cover them, too.

• Medicare – If you meet the eligibility requirements for Home Care and your doctor has certified your Homebound Status, Medicare may cover Home Care services for 60 days.
• Medicaid – Medicaid payment options depend on the policy of each state. In many cases, Medicaid covers Home Care services. You can contact your in-state Medicaid office directly to find out how your plan works with us. You can find the contact information for your state’s Medicaid program by searching the CMS.gov Contacts Database.
• Private Insurance – Most private insurance plans cover Home Care services. You can contact your insurance provider directly for your benefits and coverage. You can also give us a call to guide you through the options.
