In addition to the core therapy and palliative care programs, All Seasons offers additional supportive and specialized services that may be part of a holistic home health plan:

You will need to have seen your PCP, surgeon, or specialist 90 days prior or within 30 days after the start of care before receiving home health care. A Home Health Face-to-Face Encounter document will have to be signed by your health care provider, requesting care for you to be in a home health setting. Home Health Care requires a homebound physical status as a necessity to qualify for services.
Medicare pays 100% of the covered services and supplies. We also accept Private Pay, Medicaid, and Insurance Pay.
