• Complex Care: Specialized, tailored support for children and adults with developmental, physical, or behavioral disabilities.

Medical Complexity Disclaimer: At the CommuniCare clinic at The MAC, the Complex Care department welcomes patients with all diagnoses and disabilities, but is currently only accepting those with low to moderate medical complexity. We are not able to serve patients with intensive medical needs, such as patients who require the following:

• Hospice or palliative care
• Tracheostomy or home ventilator dependence

Appointments: A referral is required to be seen by our Complex Care Team.
